The waves are real, and they are not January 1.

Across the NPIs that carry a real, dated rate-change window in our index, the dates cluster into a handful of recurring points in the year. These are payer fee-schedule version boundaries taken from the payers' own federal filings: the date the payer's schedule itself changes for a market, not a confirmed individual contract anniversary. Not one of them is January 1.

If your billing team assumes contracts turn over on the calendar year, that assumption does not hold for the payer file boundaries we can actually see.
